An individualized program of care is tailored to meet your needs with a variety of services that provide comfort and support, while honoring your choices and preferences.
Your unique program of care will include:
- A specially trained Nurse Practitioner who provides regular, frequent visits based upon a schedule designed around your specific needs.
- A Nurse Practitioner who is on call 24 hours a day, seven days a week to provide a quick response to any medical problems that may arise.
- A care coordinator who works with nursing homes, physicians and hospitals to schedule specialist office visits, coordinate transportation, and inform the member whenever a service is scheduled.
- Social Workers who assist with a variety of issues including end of life Advanced Directives, accessing and understanding community resources and member and family support.
We're here for you Day or Night
You can count on us to maintain close communication and to respond to your needs at all times during the day or night.
- A Nurse Practitioner is available 24/7 to respond to medical problems that may arise.
- We respond to urgent calls within 15 minutes.
- The Nurse Practitioner coordinates ongoing communication with everyone involved in providing care.
- Family members receive monthly updates from the Nurse Practitioner and timely notification of any changes in the member's condition or needs.
