Our knowledgeable team of specially trained professionals works to continually maintain a personalized program of care that meets a member's unique needs. We coordinate our services to complement the care already being provided by family, friends, and the medical professionals you know and trust.
Nurse Practitioners Nurse Practitioners who specialize in the care of frail adults are the heart of the Family Choice model of care. Working in collaboration with a member's physician and nursing facility staff, they visit each member as often as weekly or more often when needed. Our Nurse Practitioners are on call 24/7 to provide orders and, if necessary, an on-site visit to assess urgent member problems. They call the member's family at least monthly and whenever there is a new care need or change in the member's medical condition.
Medical Director Oversees the overall plan of care and works collaboratively with a member's personal physician. Provides on-going clinical education to Family Choice Nurse Practitioners. Assures that a Family Choice Medical Director is available 24/7 to address Nurse Practitioners questions or concerns. Provides medical oversight of Utilization and Quality Management programs.
Associate Medical Directors Assist the Medical Director with Nurse Practitioner education and rotating call with the Medical Director to assure that a physician is available 24/7 to address Nurse Practitioner questions or concerns. Actively participates in the Quality Management Program.
Executive Director Available to answer any and all questions or concerns about the program, its employees or its operations. Oversees requests for contracts or services. Responsible for overall program administration.
Director of Clinical Services Hires, trains and manages the Family Choice Nurse Practitioner team. Works with nursing facilities and physicians to continually improve our Nurse Practitioner services and quickly resolve any problem issues.
Director of Quality Management Oversees all aspects of the Family Choice of New York Quality Management Program including providing regular Quality Indicator reports to participating facilities. Works with physician members of the Quality Management Committee to continually update the Family Choice Clinical Practice Guidelines so they reflect the most recent research in geriatric care.
Director of Utilization Management Handles all questions concerning authorization of services, clinical review criteria or contacting a physician reviewer. Works with nursing facilities to help assure proper submission and payment of claims.
Social Workers Provide support for members, families and nursing facility staff in dealing with a variety of psychosocial issues including Advance Directives, family decisions regarding the member's care, end of life concerns and accessing community resources that may provide additional help.
Care Coordinators Schedule consults, procedures and other medical services needed outside the nursing facility's setting. Also schedules non-emergency transportation.
Community Liaisons Provide education about the program and coordinate enrollment into the Independent Health Family Choice Plan.
Office Manager Available to answer questions regarding whether or not a patient is participating in the Family Choice Program.
