To enjoy the benefits of Independent Health's Medicare Family Choice HMO/SNP, a nursing facility resident must meet these requirements:
- Enroll in Independent Health's Medicare Family Choice Plan
- Reside permanently in a nursing facility which is credentialed by Independent Health and has chosen to participate in the Family Choice Program
- Currently receives medical care from an Independent Health physician who has chosen to participate in the Family Choice Plan or agrees to begin receiving care from a Medicare Family Choice Plan participating physician.
- Is eligible for Medicare Part A and B
- Continues to pay Part B premiums
- Does NOT have end stage renal disease.(This condition doesn't apply if the potential member is currently enrolled in another Independent Health Plan)
