"My mother has been a resident of a nursing facility for 6 years. Even though she receives good care at the facility and is very happy living there, her stubborn personality of non compliance has landed her in the hospital more times than we care to remember. She became weaker with every single episode. Just after mom returned from another hospital stay, I was introduced to a Nurse Practitioner from Family Choice. She informed me that she would put a plan of care into place that would hopefully prevent mom from making those critical trips to the hospital. She arranged a family meeting with mom present, stressing how improved compliance could help her. Since that meeting, mom has really been trying. Our Nurse Practitioner keeps me well informed each and every time she sees my mom, even if it is just to tell me that mom is doing ok.
I appreciate the Nurse Practitioner calling with report. Gives me a chance to discuss and explore health issues and options.
