How To Join

To enjoy the benefits of the Family Choice Program, a nursing facility resident must meet these requirements:

• Enroll in Independent Health's Medicare Family Choice Plan
• Reside permanently in a Niagara or Erie County nursing facility which is credentialed by Independent Health and has chosen to participate in the Family Choice Program
• Currently receives medical care from an Independent Health physician who has chosen to participate in the Family Choice Plan or agrees to begin receiving care from a Medicare Family Choice Plan participating physician.
• Is eligible for Medicare Part A and B
• Continues to pay Part B premiums
• Does NOT have end stage renal disease.(This condition doesn't apply if the potential member is currently enrolled in another Independent Health Plan)
