Family Choice is a unique program designed to bring more medical care, more often, right to where our members live…….in Buffalo area nursing homes.

Family Choice works in partnership with local nursing homes and physicians to address the special health and social needs of nursing home residents and their loved ones.

At Family Choice we are dedicated to:

• Identifying potential problems and illnesses before they become serious.
• Minimizing unnecessary and disruptive emergency room visits and hospital stays by providing as many medical services as possible in the member's nursing home.
• Coordinating care with a member's physician, nursing home staff, and family to provide the highest possible quality of life for those we are privileged to serve.
• Keeping the family and/or responsible party updated on the member's condition and helping them to make the best possible care decisions for their loved one.
