Independent Health's Medicare Family Choice HMO/SNP is a unique program designed to bring more medical care, more often, right to where our members live……  Western New York area nursing homes.

Family Choice works in partnership with local nursing homes and physicians to address the special health and social needs of nursing home residents and their loved ones.

• Minimizing unnecessary and disruptive emergency room visits and hospital stays by providing as many medical services as possible in the member's nursing home.
